Unknown. Maybe related to French faire gaffe (“pay attention”), or German gaffen (“to stare”).
Possibly related to the Andalusian Arab scholar and ophtalmologist of the 12th century Muhammad ibn Aslam Al-Ghafiqi, an ophtalmologist. Eyeglasses “gafas” were named after him. (Can this(+) etymology be sourced?)
